WILL APPOINTMENTS:  Will appointments are generally scheduled for Wednesday afternoons. LEGAL ASSISTANCE APPOINTMENTS:  Legal Assistance is by appointments only and are available on Tuesdays and Thursdays from 0900-1100 and 1300-1500.  Let us know the general nature of the visit when you call.

We are closed Wednesdays from 1030-1130 & the first Thursday of each month from 1200-1600.

1.  Active Duty AND Dependents                               

2.  Guard or Reserve on Title 10 orders

3.  Retirees AND Dependents                                     

4.  Foreign Military Personnel

5.  Some DoD Civilian Employees (call to check your status)

Our office will need the following to receive your claim:

            1.  SF Form 95 (Has to be signed with the amount
                 you are claiming, both individual amounts and
                 a final sum of claimed monies)

            2.  Pictures of incident

            3.  Have at least 2 quotes

            4.  Police report, if available
