Outpatient Medical Records
The 366th Medical Group maintains approximately 26,000 medical records on Active Duty Service Members, retirees, dependents, and surviving spouses. Documentation of all medical care rendered within the 366th Medical Group and results from referred care outside the facility are maintained in an outpatient medical record. In addition, the Outpatient Records Office registers records for inbound Service Members and their dependents.

Keep in mind that medical records are the property of the United States Government and originals cannot legally be released to the beneficiary.

Release of Information
The Release of Information Office is the focal point for providing copies of medical documentation. Service Members can request one full copy of their medical record, free of charge, during their military career. Additional copies can be made, but may be at the member's expense.

If you are retiring or separating from active duty, please visit the Patient Administration Office to fill out the required release form to obtain a copy of your medical record. Requests for copies of the full medical record can be submitted as early as 179 days before but not later than 60 days prior to your separation or retirement date.

If you are referred to an off-base provider, your Primary Care Manager will provide all necessary medical documentation for the appointment. If you need additional documentation, the off-base provider can request appropriate/applicable copies of you medical record via fax to (208) 828-8242 and confirm receipt at (208) 828-7503.

If you need medical documentation to file an insurance claim, the respective insurance company should contact the Release of Information Office to provide proof of their credentials and a signed authorization from the patient.

In order to protect our beneficiaries' protected health information, all requests are reviewed prior to processing to ensure the appropriate authorizations are in place.

Please remember that copy requests are not filled the same business day. Release of Information staff will inform you of the expected completion date.
