We are the Primary Care Manager, or PCM, for all active duty members stationed at Mountain Home Air Force Base that are not on flying status or air traffic controllers. We are also the PCM for dependents of active duty members stationed here, unless assigned to a PCM off base. We are also the PCM for Tri-Care Prime retirees and their families empanelled with us.
Our professional staff consists of active duty and contract civilian physicians and physician assistants and/or nurse practitioners.
Our support staff consists of active duty medical and administrative technicians, civil service registered nurses and licensed practical nurses , contract civilian RNs, LPNs and medical technicians. Most of our civilian staff are themselves retirees, prior military or dependent spouse and have extensive experience with the unique military environment.
We take pride in the healthcare we provide to those who are serving or have served their country and their dependents.
How to make an appointment you may choose either option:
1. Call the appointment line 828-7900
2. Register and book on-line at www.tricareonline.com (each person needing an appointment must be registered).
How do I get medications refilled/renewed?
There are several options available. Please allow a few days minimum to fill renew medications, so please plan ahead.
1. Medication Refill Line: 828-6947 or 800-446-8980 (if you have refills left on prescriptions filled at this pharmacy).
2. Leave a telephone message for your PCM team through the central appointment line 828-7900.
3. Mail order pharmacy: 1-877-363-1303 or www.express-scripts.com/TRICARE . For certain circumstances, this option may be the best, especially if you live far from a net pharmacy.

To make sure your health care records and test results are returned to your PCM, please follow these steps:
STEP 1: When you receive your referral authorization letter, call the provider and schedule your appointment. Please allow 10 business days to receive the letter; however, it is usually available online at www.triwest.com within a few business days.
STEP 2: After making the appointment, call TriWest at 1-866-876-2383. You will be prompted to provide the following information:
-You may select another network provider of the same "specialty" if you wish to see a provider other than the one named in your authorization letter. You can view the network providers on the directory at www.triwest.com
-Track the status of your authorizations and referrals online by registering at www.triwest.com, you can view and print out your consult within a few business days after entered by your PCM.
What if you have cancelled or changed your appointment?
Call TriWest at 1-866-876-2383 to update the referral.

For Tobacco cessation, healthy and diabetic nutritional education
We have walk-in clinic (8:30 to 10:30 a.m. and 1:30 to 2:30 p.m. Monday through Friday) for blood pressure checks, throat cultures, wart removals - after initial evaluation by PCM, administration of ordered shots, shaving waivers - after evaluated by a provider (if the current waiver is older than one year, an appointment is required to obtain a valid waiver), wound checks and suture removal - after evaluated by a doctor.
You may always leave a message for your PCM via the nurse telephone consult. Call 828-7900 and follow the options. If in doubt, talk with the booking clerk and the professional staff at the Tri-West will assist you.
